Community Public Health Teams (CPHT) is a project that brings
together community-based organizations, health care partners,
and Public Health staff. These teams work at the household level,
conducting home visits to gather health needs and connect
people to care. These teams also work at the community level,
inviting people to participate in creating community action plans.

Herald Christian Health Center is a non-profit
community clinic that provides medical care and social
services assistance to uninsured, low-income,
undocumented and underserved populations in the
San Gabriel Valley.
CORE is a global humanitarian aid organization that
brings immediate aid and long-term recovery solutions
to underserved communities in 27 regions worldwide.

reported need, with 173 households
requesting food support, showing
that access to affordable and
nutritious food is one of the most
urgent challenges facing families in
our community.

The Coalition Model
To address food insecurity needs, we created a community network (also known as
coalition) where community members, health partners, nonprofit organizations, DPH, and
local partners can work together to improve food access, share resources, and respond
to the needs residents are experiencing.
