in the schedule of bid items above, within the limits define in Section 3 -2.2.1 of 1heStandard
Specifications, to stay within the budgeted amount of this project.
The undersigned agrees that these Contract Bid Forms constitute a firm offer to the Owner which
cannot be withdrawn for the number of Working Days indicated In the Notice Inviting Bids from and
after the bid opening date, or until a Contract for the Work is fully executed by the Owner and athird
party, whichever is earlier. The undersigned also agrees that if there is a discrepancy between the
written amount of the Bid Price and the numerical amount of the Bid Price, the written amount shall
govern.
Attached hereto is a certified check, a cashier's check or a bid bond in the amount of
10% OF BID AMT Dollars ($!J5 569.70 said amount being not less than ten percent (106,0) of the Total
Bid Price. The undersigned agrees that said amount shall be retained by the Ownerif, upon award,
we fall or refuse to execute the Contract and furnish the required bonds, certificates and
endorsements of insurance and other certifications within the time provided.
If awarded a Contract, the undersigned agrees to execute the formal Contract, which will be
prepared by the Owner for execution, within five (5) Calendar Days following the Letter of Award for
the Contract, and will deliver to the Owner within that same period the necessary original Certificates
of Insurance, Endorsements of Insurance, Performance Bond, Payment Bond and all other
documentation and certification required by the Contract.
The undersigned offers and agrees that if this bid is accepted, it will assign to the purchasing body
all rights, title, and interest in and to all causes of action it may have under Section 4 of the Clayton
Act (15 U.S.C. Section 15) or under the Cartwright Act (Chapter 2 of Part 2 of Division 7 of the
Business and Professional Code), arising from purchases of goods, materials or services by the
Bidder for sale to the purchasing body pursuant to the bid. Such assignment shall be made and
become effective at the time the purchasing body tenders final payment to he Contractor_
Bidder understands and agrees that, when requested by Owner, he shall provide: (1) evidence
satisfactory to the Owner of Bidder's California contractor's license(s) in good standing; (2) evidence
that the person signing this Bid is authorized to bind Bidder to this Bid and to a contract resulting
therefrom; and (3) any other information and documentation, financial or otherwise, needed by
Owner to award a Contract to the lowest responsible and responsive bidder.
Bidder understands and agrees that liquidated damages shall applyto this Contract in the amounts
of five hundred dollars ($500.00) if project is not completed in forty -five (45) calendar days. The
Contract Time will begin to run ten (10) Working Days from the date of the Notice of Proceed and
subject to the terms and conditions described in the Contract Form and the Contract Documents.
Bidder acknowledges that data submitted with its Contract Bid Forms which it requires to be
Incorporated into a Contract arising out of this Submittal has been so identified by Bidder. Bidder
further acknowledgos that the Owner may, at Its discretion, incorporate any of the remaining data
submitted herewith into a contract arising out of this Bid.

2.6 LIST OF PROPOSED SUBCONTRACTORS
In compliance with the "Subletting and Subcontracting Fair Practices Act," Sections 4100
through 4114 of the California Public Contract Code, and any amendments thereto, each
Bidder shall provide the Inforrnation requested below for each subcontractor who will
perform work, labor or render service to Bidder in or about the construction of the Work in an
amount in excess of one -half of one percent (greater than 0.5 %) of the Bidder's Total Bid
Price, or, in the case of bids or offers for the construction of streets or highways, including

3. PRODUCT DESCRIPTION
Basic Use: The Duro -Last Single -Ply
Roofing System is a prefabricated
waterproofing system used in the
following applications:
• Re -cover over existing systems
• New construction
• Both combustible and
noncombustible decks
Limitations: Duro -Last membrane
must not be installed over assemblies
with saturated under -lying insulation or
directly over existing incompatible roofs
without proper separation.
Composition and Materials: Duro-
Last membrane is a proprietary
thermoplastic formulation consisting of
resins, plasticizers, stabilizers,
fungicides, flame- retardants, and U.V.
absorbers, which is laminated to a weft -
inserted polyester fabric. It is available
in white, tan, gray, dark gray, terra
cotta, Rock Ply, slate gray and
sandstone Shingle Ply.
Sizes: For ease of handling and
reduced field seams and faster
installation time, Duro -Last, Inc. factory
welds material in large sheets up to
2,500 ft2 (232 m2) as specified by
architects, contractors, or installers.
The system weighs approximately
0.32 lb. per ft (1.28 kg per m).
Accessory Components: Duro -Last,
Inc. produces a complete line of
standard and custom - fabricated
accessories. These accessories are
made from the same material as the
deck membrane and are made in a
Duro -Last factory, not on the roof.
Standard Accessories: Round pipe
flashings from 1 inch (2.5 cm) to 15
inch (38 cm) in diameter, inside and
outside corners, two -way roof vents,
roof drain liners, gravel /water stop with
factory welded Duro -Last skirts,
termination bar, and fascia with cover
and joint clips.
Custom- Made Accessories: Round
pipe flashings greater than 15 inch (38
cm), parapet wall flashings, scuppers
and curbs for all types of roof
penetrations. (For example: HVAC
units, skylights, chimneys, etc.) If there
is a need for a custom flashing that has
not been mentioned, provide a drawing
and Duro -Last will manufacture it.

INSTALLATION
Duro -Last, Inc. provides detailed
application instructions and training for
roofing contractors and consulting
services to architects, engineers, and
specifiers. The following are general
instructions for installing the Duro -Last
Roofing System.
Deck Surface Preparation: The
substrate on which Duro -Last
membrane is to be applied must be
clean, smooth, free of sharp edges,
loose debris, and standing water.
Gravel should be leveled. All surface
preparations must be done in strict
accordance with the Duro -Last
application specifications.
Insulation: Insulation must be laid on
the deck and cut neatly around all
penetrations. It must then be attached
with approved fasteners according to
the Duro -Last specifications. Do not lay
more insulation than can be covered in
the same day. Stagger the insulation
boards 50% from row to row. No gaps
to exceed 114 inch.
Placement., Using the roof layout
diagram, select the proper marked and
rolled premanufactured sheet of Duro-
Last membrane. Orient the membrane
so the fastening tabs are perpendicular
to the ribs or corrugations of steel deck,
or to the width of prestressed concrete
"T" slabs. Secure the membrane to the
roof deck through the fastening tab with
approved fasteners and plates, in
accordance with Duro -Last
specifications. Unfold the sheet,
thus covering the fastening tab.
Continue unfolding until the next
fastening tab appears. Fasten this tab
to the deck and repeat the above
process until the complete sheet is
installed.
Penetrations: During the
aforementioned procedure, make
cutouts in the membrane for
protrusions. Fasten around cutouts
with Duro -Last approved fasteners. The
skirt of the factory- fabricated
accessories when welded to the deck
membrane will cover these cutouts.

This Spec -Data sheet conforms to
the editorial style prescribed by

Institute. The manufacturer is
responsible for technical accuracy.
Breather Vents: One Duro -Last two -
way breather vent must be installed for
each 1,000 ft2 (90 m2).
Parapet Walls: Duro -Last custom
fabricated parapet wall flashings are
designed for each individual
application. Secure through the
fastening tab at the bottom of the wall
using approved fasteners and plates.
The factory- welded skirt will cover the
fasteners when it is heat welded to the
deck sheet.

7. COMMERCIAL WARRANTIES
Upon the successful completion of a
final inspection by a Duro -Last quality
assurance specialist, Duro -Last, Inc.
will furnish to the Duro -Last contractor
either a 15 -year or 20 -year warranty.
The 15 Year NDL Warranty covers
material and labor for repair or
replacement for the full 15 years.
The 20 Year Limited Warranty
covers both material and labor for
repair or replacement for the first 10
years. The second 10 years is
prorated and covers material only.
8. MAINTENANCE
Keep the finished roof free of debris.
Do not use roof area for storage. No
other maintenance is required.
9. TECHNICAL SERVICES
Duro -Last, Inc. provides technical
information and roof membrane layout
services to architects, engineers, and
licensed contractors for specifying and
detailed installation of the Duro -Last
Roofing System for various field
applications. Technical brochures,
manuals, and specifications are
available upon request.

Description:
Duro - Last ®, Inc. requires the use of Duro- Coated
Screws for mechanical fastening of the Duro -Last
roofing system. Duro -Last HD Screws are #14
diameter, are equipped with a P -3 truss head and are
available with either a drill point or a spade point.
Duro -Last HD Screws can be used in most types of
roofing applications. These Duro- Coated Screws must
pass twice the FM 4470 standard for corrosion
resistance, totaling 30 cycles in a Kesternich cabinet.
Duro -Last, Inc. also requires that all Duro- Coated
Screws pass 200 and 401 bend tests (hardness) and
be able to penetrate a 20 gauge deck in under three
seconds with a 50 Ib. (222 N) load.
Installation:
1. Penetration into all approved deck types is 1 inch
(25 mm) minimum, from the top surface of the
deck.

Description
DensDeck® Roof Board is an exceptional fire harrier, thermal barrier, coverboard
and recovery board used in various commercial rooting systems. The DensDeck
Roof Board design employs fiberglass mats front and back that are mechanically
bonded to a high density gypsum core, providing excellent fire resistance and
wind uplift properties. The unique construction of DensDeck Roof Board provides
superior flute spanning that stiffens and provides increased foot traffic resistance
to the roof deck. Additionally, DensDeck Roof Board has been shown to withstand
delamination, deterioration and job -site damage far more effectively than roofing
membrane substrates such as paperfaced gypsum board, fiberboard and perlite
insulation. DensDeck Roof Board is highly resistant to the growth of mold when
tested, as manufactured, per ASTM D 3273.
Primary Uses
Roof system manufacturers and designers have found DensDeck Roof Board to be
compatible with many types of mofing systems, including: built -up, modified bitumen,
single ply, metal systems, wood shingle and shake, tile, slate, as well as a recovery
board and everlayment protection board for polyisocyanumte and polystyrene insulation.
DensDeck Roof Board can also be used as a form board for poured gypsum concrete
deck in roof applications as well as a substrate for spray foam roofing systems. 112"
(12.7 mm) and 5 /8" (15.9 mm) Dens0eck Roof Board may also be used in vertical
applications as a backer board or liner for the roof side of parapet walls.
Some membrane manufacturers have hot mop asphalt or torch applications directly
to DensDeck Roof Board without using a primer or base sheet. Consult with the
system manufacturer for their recommendations with this application. DensDeck
Roof Board is the preferred substrate for vapor retarders.
Standards and Code Approvals
DensDeck Roof Boards are manufactured to meet AST "M C 1177 and have the
following approvals:
• Florida Product Approval Code FL 1250
• Miami -Dade County, Florida NOA 08- 0908.10
Recommendations and limitations
DensDeck Roof Boards are manufactured to art with a properly designed roof system
following good roofing practices. The actual use of DensDeck Roof Board as a roofing
component in any system or assembly is the responsibility of the roofing system's
design authority. Consult with the appropriate system manufacturer and /or design
authority for system and assembly specifications and instructions on applying other
products to DensDeck Roof Board. Georgia- Pacific does not warrant and is not
responsible for any systems or assemblies utilizing DensDeck Roof Board or any
component in such systems or assemblies other than DensDeck Roof Board.
The need for a separator sheet between the DensDeck Roof Board and the roofing
membrane must be determiner) by the roof membrane manufacturer or roofing
system designer.

Confirm any priming requirements with the membrane manufacturer. When applying
solvent -based adhesives or primers, allow sufficient time for the solvent to flash off
to avoid damage to roofing components.
DensDeck Beef Boards should not be subjected to abnormal or excessive loads or
foot traffic, such as, but not limited to, use on plaza decks or under steel- wheeled
equipment that may fracture or damage the panels. Provide suitable roofing system
protection when required.
When using DensDeck Roof Boards for hot - mopped applications, Georgia- Pacific
recommends maximum asphalt application temperatures for Type III asphalt of
4251(218 °C) to 450 °F (232 °C). Application temperatures above these recommended
temperatures may adversely affect roof system performance. For application temper-
atures in excess of 4501(232 °C) and for mopping of type IV asphalt, ribbon or spot
mopping or the installation of a perforated base sheet are recommended methods of
bonding asphalt in lieu of full mopping. Consult and follow the roofing system manu-
facturer's specifications for full mapping applications and temperature requirements.
Conditions beyond the control of Georgia- Pacific, such as weather conditions, dew,
leaks, application temperatures and techniques may cause adverse effects with
roofing systems.
Moisture Management
DensDeck Roof Boards, like other components used in roofing systems, must
he protected from exposure to moisture before, during and after installation.
Remove the plastic packaging from all DensDeck Roof Board immediately upon receipt
of delivery. Failure to remove the plastic packaging may result in entrapment of con-
densation or moisture. DensDeck Roof Board stored outside must be stared level and
off the ground and protected by a breathable waterproof covering. Provide means for air
circulation around and under stored bundles of DensDeck Roof Board. DensDeck Roof
Board must be covered the same day as installed.
Avoid application of DensDeck Roof Boards during rain, heavy fog and any other condi-
tions that may deposit moisture on the surface, and avoid the overuse of non - vented,
direct -fired heaters during winter months. When roofing systems are installed on new
poured concrete or light weight concrete decks or when re- roofing over a wet existing
concrete deck, a venting base sheet or vapor retarder should he installed above the

Always consult the roofing system manufacturer or design authority for specific
instructions far applying other products to DensDeck Roof Boards.
Moisture vapor movement by convection must be eliminated, and the flow of water
by gravity through imperfections in the roof system must he controlled. After a leak
has occurred, no condensation on the upper surface of the system should be tolerated,
and the water introduced by the leak must be dissipated to the building interior in a
minimum amount of time.
Although DensDeck Roof Boards are engineered with fiberglass facings and high density
gypsum cores, the presence of free moisture can have a detrimental effect on the
performance of the product and the installation of roofing membranes. For example,
hot asphalt applications can blister; torched modified bitumen may not properly bond;
and adhesives for single ply membranes may not dry properly. Moisture accumulation
may also significantly decrease wind uplift and vertical pull resistance in the system or
assembly. DensDeck Roof Boards containing excessive free moisture content may
need to be evaluated for structural stability to assure wind uplift performance.

DensDeckh' Roof Boards are excellent fire barriers over combustible and noncombustible
roof decks, including steel decks.
VL 790Ciassideation. DensDeck Roof Boards have been classified by Undervvriters
Laboratories (UL) for use as a fire barrier over combustible and noncombustible decks
in accordance with the ANSINL 790 test standard. The UL classification includes a
comprehensive Class A, B or C rating. For additional information concerning the UL 790

UL 1256 C1assifcan3n. DensDeck Roof Boards have also been classified by UL in roof
deck constructions far internal (under deck) fire exposure in accordance with the
