locations for City of Rosemead listed in the RFP 2014-13.
Dedicated has extensive public agency experience providing custodial services to City of Los Angeles, City of Palm Desert
and UCLA Athletics. In addition, our management team has work with other public agencies such as AQMD, City of
Tustin,City of Inglewood,City of Santa Monica, Los Angeles County Internal Service Department and Los Angeles County

Overview
Dedicated Building Services, LLC, ("Dedicated") was formed on April 5, 2013 and commenced its first account
(Baldwin Hills Crenshaw Plaza) on May 1, 2013. Dedicated is a non-union company that specializes in
commercial janitorial services, pressure washing, parking lot cleaning and building engineering services. In
less than two years, Dedicated has grown to over 242 full time employees and 96 part-time employees.
Currently, Dedicated provides building maintenance services to large accounts such as City of Los Angeles,
UCLA Athletics, Baldwin Hills Crenshaw Plaza, Metro Centre, Los Angeles Housing Authority and UCLA's Pauley
Pavilion. While Dedicated as a company is relatively new, it is important to note that our founder, Derek C.
Smith, brings over 30 years of industry experience including 28 years working with public agencies throughout
southern California. Mr. Smith was the founder and former President/CEO of Diamond Contract Services Inc.
("Diamond") who had provided custodial services to public agencies such as City of Los Angeles, City of Tustin,
City of Santa Monica, City of Inglewood, Los Angeles International Airport, Los Angeles County Internal
Services Department, Southern California Air Quality Management District Headquarters, County of Los
Angeles Public Libraries, Orange County Transportation Authority(OCTA)and the Natural History Museum just
to name a few. He left Diamond in February 2013 and started Dedicated.

services, pressure washing services, parking lot cleaning, carpet cleaning and floor care services. The

Additionally, Dedicated and its management team have proven their ability to staff large projects in
very short time periods. In October 2013, Dedicated started the UCLA Athletics contract with two
weeks notices and in June 2014, the City of Los Angeles gave Dedicated 48 hours' notice to begin
services for its 180 plus locations. Finally, over the past two years, Dedicated has managed large staff
transitions having recruited over 175 employees to clean approximately 2,800 student apartments at
UCLA. Some of these employees are now part of our permanent full and part time staff.

Dedicated provides custodial services 5, 6 and 7 days per week to 189 locations for the City of Los
Angeles. Our facility types encompass over 65 Maintenance and Service Yards, 72 Libraries, 41
Municipal Office Buildings and 22 Community Centers as well as other city owned buildings. Our scope
of service includes all office areas, conference rooms, lunchrooms, hallways, stairwells, locker rooms,
restrooms, exterior entrance areas and parking lots. Our services include daily custodial services,
weekly parking lot sweeping, monthly pressure washing services, monthly window cleaning services
and quarterly floorcare including strip and wax and carpet cleaning. Dedicated's staff includes 159
custodians, 8 floorcare technicians, 4 pressure washers and 2 window cleaners. Similar to the UCLA
contract discussed below, Dedicated proved its ability to successfully manage a rapid transition plan.
The City of Los Angeles gave Dedicated 48 hours to start this contract as the previous contractor,
Diamond Contract Services, ceased operations on June 6th. Dedicated recruited and hired the existing
crew on June 7th and 8th, ordered new equipment and supplies for delivery on June 9th and all locations
were covered and serviced on Monday, June 9th. Dedicated is now developing cleaning assignment
schedules and inspection programs which we implemented on July 1't.

Baldwin Hills Crenshaw Plaza is a recently $35 million dollar renovated shopping mall with over
850,000 square feet of retail space including three parking structures and four parking lots. Dedicated
provides services 7 days per week including all holidays. Our services include janitorial, parking lot
sweeping, pressure washing services, floor care services and building engineering services. Dedicated's
staff includes 1 Project Manager, 3 Supervisors, 35 cleaners, 1 Chief Engineer and 3 maintenance techs.
Dedicated has worked with the property management company, Primestor, to consistently improve
and maintain the new appearance of the shopping center. Our team is regularly recognized by
management and patrons for their outstanding customer services and facility cleanliness.

Easton Stadium, Yates Center and GIFF. Our scope of services encompasses over 300,000 square feet
of cleaning space include all office areas, conference rooms, lunchrooms, hallways, stairwells, Hall of
Fall Center, restrooms, locker rooms, medical offices, training areas, weight rooms and exterior
entrance areas. The breath of our services includes daily custodial services, weekly floorcare including
strip and wax, spray buff and carpet cleaning, and monthly window cleaning services. Dedicated's staff
includes 1 Project Manager, 1 Asst. Manager, 9 custodians and 2 floorcare technicians. With this UCLA
contract, Dedicated proved its ability to successfully manage a rapid transition plan. Dedicated was
given two weeks to start this contract as the previous contractor, Pride Industries, resigned after two
months of unsuccessful services. Dedicated assembled a new crew, ordered new equipment,
developed cleaning assignment schedules and inspection program all within the two week time period.

Dedicated has two separate contracts with the City of Palm Desert: Public Works and Parkview Office
Complex. The Public Works contract provides custodial services 5 days per week to City Hall,
Corporate yard, Visitor Center and Portola Community Center. The Parkview Office contract provides
custodial services to two office buildings total approximately 40,000 square feet. Our scope of service
for both contracts includes all office areas, conference rooms, lunchrooms, hallways, stairwells, locker
rooms, restrooms, exterior entrance areas and parking lots. Our services include daily custodial
services and weekly parking lot policing. Dedicated's staff includes 1 day Porter and 4 custodians.

2. Proposed Staffing and Project Organization
Overview
Dedicated has assembled a strong management team to ensure that the daily, weekly, monthly and quarterly
tasks are performed in accordance to the Request for Proposal specifications. Our team is led by Mr. Derek C.
Smith and Edward Beasley. Mr. Smith will serve as the Contract Manager and ensure that the day to day
operations are being performed. Mr. Beasley will coordinate the periodical schedules for floor care, carpet
cleaning and window cleaning as well as work with our Area Supervisor, Raul Villanueva. Mr. Raul Villanueva
will operate as our Project Manager. He will be responsible for the day to day operations that are being
performed at the City of Rosemead. In addition, Dedicated will assign Brittany Ramos as the Account
Executive ("AE") who will be responsible for conducting monthly inspections at the various City of Rosemead
locations. These inspections will be used to determine where Dedicated's training might be deficient or what
additional training subjects should be covered. The AE will provide the results of these inspections weekly to
the City of Rosemead Manager with a summarized monthly report by location. Both the Mr. Villanueva and
Ms. Ramos will attend the weekly meeting with City of Rosemead representative.
Dedicated will develop work tasks assignment sheets ("WATS") to ensure that all required work is performed
and documented. Dedicated has analyzed the specification requirements and determined that the cleaning
production rate at each location should be based between 3,000 to 3,500 square feet per hour. The
production rate is based on industry standards for office cleaning as well as our empirical knowledge.

a) Organization Chart:
Dedicated has developed an Organization Chart for City of Rosemead based on several factors:
previous public agency experience, changes in the scope services from previous contract and
compliance with local regulations.
Dedicated must comply with California Law SB20, California Janitor Displacement Act, which requires
that Dedicated offer employment to all employees currently working at City of Rosemead. If the
current employees fail to pass our background checks, Dedicated does have current employees who
live in Rosemead, San Gabriel, Pasadena and Montebello. Moreover, given that Dedicated's Project
Manager, Raul Villanueva lives in West Covina our transition should be smooth and seamless. Below
is the organizational chart for how Dedicated will staff and operate each City of Rosemead location.

b) Key Personnel:
Dedicated's key personnel for this project are Derek Smith, Ed Beasley, Raul Villanueva and Brittany
Ramos. Mr. Smith will be designated as Dedicated's Contract Manager. As Contract Manager,
Currently, Mr. Smith operates in this capacity for other Dedicated accounts listed in our references. He
has been employed with Dedicated for 19 months and has over 30 years of industry experience in
senior leadership roles. His understanding of the industry is virtually unmatched and will once again be
a valuable resource and partner to City of Rosemead. Mr. Beasley brings significant project
management and managerial experience throughout his 20+year career. He has over 5 years of direct
experience including working specifically with public agencies while employed at Diamond. Mr.
Villanueva will be designed as Dedicated's Project Manager, he has the overall responsibility to ensure
all personnel, assignments, training, supplies, equipment and work schedules are supplied to each City
of Rosemead facility. Mr. Villanueva will be at each facility at minimum of once a week and will
allocated approximately eight (8) to ten (10) hours per week to City of Rosemead. He brings over 20
years of industry experience including building engineering. Dedicated will assign Brittany Ramos as
Account Executive to perform monthly inspections at the various City of Rosemead facilities. . These
inspection reports will be provided to the responsible City of Rosemead contact. In addition, Ms.
Ramos will provide a summarized monthly report for all inspections performed during the month.
As required, Ms. Ramos will attend weekly meetings along with Mr.Villanueva.

3. Technical Approach/Work Plan
Overview
Dedicated will develop work assignment tasks sheets and daily, weekly and monthly checklist schedules as
well as monthly and quarterly project work schedules to comply with every aspect of the scope of work
indicated in the RFP. Dedicated is committed to developing a work force that has the "Dedicated Spirit" to
meeting the goals and objections of City of Rosemead.
Our approach to performing the daily services is based on the production standards set forth in Section B and
team cleaning approach to accomplish the tasks in each building. Our Account Executive ("AE"), Brittany
Ramos will perform monthly inspections. Ms. Ramos will utilize inspection forms with satisfactory and
unsatisfactory ratings and a passing scoring is 93% or higher. Any facility not receiving a 93% will be re-
inspected the next day to assure that any deficiencies noted were corrected.
There are no special issues or problems that are likely to be encountered in this project and we have included
some proposed enhancements to the service delivery to ensure that communication between Dedicated and
City of Rosemead is fluid and consistent.
a) Approach to Scope of Work:
Our Work Plan includes work assignment tasks schedules for each team member assigned to City of
Rosemead. The semi-annual floor care will be completed by two floor care technicians. The crew will
complete one location at a time before moving to another location. The quarterly high dusting will be
performed by a separate two person crew as well as quarterly window cleaning services.

Labor Resources by Location:
Dedicated has thoroughly analyzed the individual task cleaning requirements and developed a staffing
plan based on industry cleaning production standards as well as our previous almost 30 years working
with public agencies. For City Hall and the Community Centers, the cleaning production rate is 3,500
square feet per hour and Public Safety Center, the production rate is 3,000 square feet per hour. For
parks, our productions standards were based on per restroom per cleaning as well as the other areas
including in the RFP. Below are our hourly labor resources allocations by location:

c) Quality Control, Budget and Schedule Control
As mentioned previously, inspections sheets will be utilized for quality control purposes as well as
training guides. Based on the schedules provided in the RFP, all work will be scheduled daily, weekly,
monthly and quarterly. Dedicated will provide written documentation to the designated City of
Rosemead official. Monthly, Dedicated will provide a quality control report that summarizes all
inspections for the month by location showing the overall score.

d) Proposed Enhancements and Procedural Innovations:
a) Area Supervisor shall instruct employees not to disturb or move items on the desks; open
drawers or cabinets; use telephones; touch computer equipment or other office equipment and
devices. We shall replace any damaged items at no cost to City of Rosemead.
b) Area Supervisor will ensure that all cleaning supplies will be stored and used in a way as to
prevent any damage or harm to persons,equipment or furnishings.
c) Area Supervisor shall report immediately via email if an alarm is not turned off or is
malfunctioning or tampered with.
d) Cleaning crew shall follow our checklist of daily, bi-weekly, weekly, monthly duties, quarterly
and semi-annual and turn in a signed copy to Area Supervisor every week to ensure that all
work was performed.
e) Cleaning crew shall not leave any office, suite or building unlocked or otherwise unsecured
while unattended at anytime throughout the custodial cleaning process.
f) Area Supervisor shall report any conditions of leaky faucets, stopped toilets and drains, broken
fixtures, building damage etc.to the City of Rosemead contact via email within a timely
manner.
g) Area Supervisor will ensure that our employees will not use radios without headphones while
working in the facility.
h) Area Supervisor will not allow our employees to loiter before or after their working hours.
i) Area Supervisor will be on site during crew work schedule. He/She must be able to read,write,
speak and understand English.

4. Exceptions/Deviations
There are no exceptions to or deviations from the requirements of this RFP. Dedicated does not wish
to propose alternative approaches to meeting the Agency's technical or contractual requirements.

I. The contractor agrees that for requested and/or required changes in the scope of
work, including additions and deletions on work not performed, the contract sum
shall be adjusted in accordance with the following unit prices.
II. Contractor is advised that the submitted unit prices will be used as one of the
determining factors in the contract award. Unreasonable prices may result in
rejection of the entire bid proposal. Unit prices listed below refer to all services
provided, including but not limited to, materials, labor, overhead, and profit for the

Note: The City of Rosemead reserves the right to reduce or increase the quantities of any items
in the schedule of bid items above, within the limits define in Section 3-2.2.1 of the Standard
Specifications, to stay within the budgeted amount of this project.
The undersigned agrees that these Contract Bid Forms constitute a firm offer to the Owner
which cannot be withdrawn for the number of Working Days indicated in the Request for
Proposals from and after the bid opening date, or until a Contract for the Work is fully executed
by the Owner and a third party, whichever is earlier. The undersigned also agrees that if there
is a discrepancy between the written amount of the Bid Price and the numerical amount of the
Bid Price, the written amount shall govern.
If awarded a Contract, the undersigned agrees to execute the formal Contract, which will be
prepared by the Owner for execution, within five (5) Calendar Days following the Letter of Award
for the Contract, and will deliver to the Owner within that same period the necessary original
Certificates of Insurance, Endorsements of Insurance, Performance Bond, and all other
documentation and certification required by the Contract.
The undersigned offers and agrees that if this bid is accepted, it will assign to the purchasing
body all rights, title, and interest in and to all causes of action it may have under Section 4 of the
Clayton Act(15 U.S.C. Section 15) or under the Cartwright Act (Chapter 2 of Part 2 of Division 7
of the Business and Professional Code), arising from purchases of goods, materials or services
by the Bidder for sale to the purchasing body pursuant to the bid. Such assignment shall be
made and become effective at the time the purchasing body tenders final payment to the
Contractor.
Bidder understands and agrees that, when requested by Owner, he shall provide: (1) evidence
that the person signing this Bid is authorized to bind Bidder to this Bid and to a contract resulting
therefrom; and (2) any other information and documentation, financial or otherwise, needed by
Owner to award a Contract to the lowest responsible and responsive bidder.
Bidder acknowledges that data submitted with its Contract Bid Forms which it requires to be
incorporated into a Contract arising out of this Submittal has been so identified by Bidder.
Bidder further acknowledges that the Owner may, at its discretion, incorporate any of the
remaining data submitted herewith into a contract arising out of this Bid.
The undersigned acknowledges receipt, understanding and full consideration of the following

JANITORIAL SERVICES FOR VARIOUS CITY FACILITIES
Clarifications/Responses for RFP
1. Current Contract and Pricing
Current Monthly Prices and Current Contract are included as PDF attachments.
Scope of work included in current request for proposal may vary from the scope
of work included in existing contract.
2. Wages
The City does not have a living wage requirement. Per California State Code, this
contract for janitorial services is not subject to prevailing wages.
3. Proposal Submittal
Contract Bid Forms, which are included in the RFP documents, must be
submitted as a part of the proposal. Contractors may submit any additional
documentation desired, however, all Contract Bid Forms Sec. 1-3 (included in
RFP documents) must be submitted.
Scope of Work Clarifications
1. Trash Removal —All sites maintain dumpsters for the disposal of refuse (trash)
generated within that facility. As part of the orientation to the new contract,
contractor will be provided access to these dumpster facilities. All segregated
recyclables that are collected from the blue bins in City facilities must be
deposited in the White Recycle dumpsters located at any of the three following
sites:
• City Hall
• Garvey Community Center
• Rosemead Park
2. Restroom Supplies — Per the RFP documents, selected contractor is to supply
all necessary restroom consumable products, including, but not limited to toilet
paper, paper towels, urinal cartridges, etc. (Specification is included in RFP
document). Although the City is unable to provide a quantity of products
consumed through the course of this contract, further clarification is provided.
a. Stocking: It is expected that all provided dispensers (toilet paper, paper
towels, soap, etc.) will be filled to capacity at every visit. The City reserves
the right to determine whether the contractor is sufficiently stocking all
necessary products.
b. Paper Towels / Hand Dryers: Several restrooms throughout the City
utilize hand dryers. Contractor is not required to provide paper towels in
restrooms that contain hand dryers. Proposers shall verify quantity of such
restrooms if desired.
c. Urinal Cartridges: The contractor is responsible for providing necessary
urinal cartridges for waterless urinals. The City of Rosemead has
approximately 20 waterless urinals.
3. Clarification Regarding Cleaning Kitchen Surfaces: Reference RFP:
E. Dust removal and fixture cleaning - Low dusting is the removal of all dust, lint, litter,
and dry soil from the surfaces of desks, chairs, file cabinets, book shelves, tables, and
other types of office furniture and equipment from horizontal ledges, window sills,window
coverings, hand rails, etc. to a line 7'g" above the top of the floor level. Vent grills, within
these limits, shall be dusted on a weekly basis. A level of care shall be taken to complete
dust removal services without disturbing personal work areas.
I. Kitchens — Some facilities have kitchenettes and two facilities have full kitchens. On
each site visit, kitchen counters and sinks shall be wiped free of debris, trash shall be
removed and floors shall be tended to.
4. Clarification Regarding Cleaning Vending Machines: Reference RFP:
H. Clean All Fixtures (i.e. drinking fountains, telephone booths, counters, etc.). After
cleaning, all fixtures shall be free from streaks, stains, spots, smudges, scale, and other
obvious soil.
Facilities Clarifications
1. SCHEDULING/HOLIDAYS
City facilities close in observance of the following holidays: New Year's Day,
Martin Luther King Day, President's Day, Memorial Day, July 4th, Labor Day,
Veteran's Day, Thanksgiving, and Christmas EXCEPT for City Parks. City parks
operate 365 days a year and must be cleaned daily.
2. City Hall: City Hall to be cleaned Sunday — Thursday (5X per Week). The
provided facility description for City Hall indicates that it operates Monday
through Thursday, and as such is to be cleaned Monday through Thursday.
Although the building is not open for public access Friday-Sunday, often times it
is used. City Hall shall also be cleaned on Sunday Evenings to ensure the
desired appearance when it opens for business on Mondays.
3. Parks — Services requested in parks pertain only to buildings/facilities explicitly
listed in facility descriptions within request for proposal documents. Trash
removal shall only be done within these buildings/facilities as general trash
removal within parks is not included in this scope of work.
4. Garvey Community Recreation Center: Regarding the high cleaning and upper
window cleaning:
• High Cleaning shall be performed per the frequencies included in the
specifications. All hanging light fixtures shall be dusted as a part of the
quarterly high cleaning.
• Upper Window Cleaning:
o Frequency of cleaning for upper windows, in main lobby, shall be
reduced to 2 X per year. (See Picture Below)
