EXPERIENCE
UNISERVE currently delivers janitorial services daily to more than 15 million square feet of
commercial buildings and properties. We bring to each client the expertise and experience of
more than 48 years with a full line of facility maintenance services from janitorial to window
washing services, pressure washing, parking lot maintenance and grounds keeping including
landscaping. UNISERVE employs in excess of four hundred employees.
Title Profile
CEO/Chairman Sam Hwang, in addition to his forty-seven years
experience in providing janitorial services,Sam has been
continuously managing janitorial services at municipal
locations since 1976.
Chief Operating Officer Anthony Santana has been providing continuous
management of janitorial services since 1997. More
specifically, Anthony has been directly responsible for
services at multiple locations throughout California and
the Pacific Northwest. Anthony successfully
implemented our services to clients such as the City of
Spokane and the City of Westlake Village.
Vice-President James Frazier has over thirty years experience in
managing large scale janitorial projects. His
responsibilities have included numerous airport
terminals at Los Angeles and Ontario International
Airports.
Director Beginning in 1991, Eugene Hwang has been involved in
managing janitorial operations for our Fortune 1000
clients. In regards to FedEx, he has been involved with
FedEx sites from 1992 to the culmination of the janitorial
strategic sourcing initiative in 2003.

restaurant, a 120 seat lecture hall, 5 interconnected museum galleries and 2 subterranean parking
structures which have 1,200 parking spaces. UNISERVE provides complete Janitorial and Day Porter

Spokane County has chosen to make UNISERVE their prime contractor for janitorial and
housekeeping services. Services include, but are not limited to, all phases of janitorial and
housekeeping services and other related work. Services are provided to approximately 26 county
buildings and sites totaling over 550,000 square feet. UNISERVE provides services to the following
for Spokane County:Adult Detention (Bio hazard cleaning and cell disinfecting—on call services),
Public safety, Human Resources, Juvenile detention, Courthouse & Annex, Community services,
Public Works, Emergency services, Elections office, Information Systems, Sheriff's department,
Public Defenders and Engineering. Window cleaning is performed inside and out. All wastewater

TECHNICAL APPROACH AND METHODOLOGY
The account representative assigned to this project will be responsible for the overall management
of the City of Rosemead work locations. In addition to those duties, the account representative
will at the minimum, visit each location weekly to meet with the client to address any concerns and
to provide a weekly recap of service activities.
UNISERVE Facilities Services operates 24/7/365. During hours outside of normal business hours,
customers may contact their account manager. Our call back times will be within a threshold of
five to ten minutes. Our emergency response times to the City of Rosemead facilities will be within
one hour. Upon award, UNISERVE will provide the City of Rosemead with emergency contact
information which can be by phone, email, internet and cellular phone messaging(SMS).

An effective training program is central to the success of our Green Cleaning program. We are
committed to ensuring that every associate, supervisor and operations manager understands the
basis of our Green Cleaning program. Beyond this, we provide training to our support staff and
corporate personnel as well. All new associates are thoroughly trained in the use of all tools,
equipment and chemicals they will use in the performance of their specific job duties. They will
also undergo an extensive safety training program which will also be documented for your review.
Initial Assignment Training takes place on-site under the direction of the UNISERVE supervisor
and/or manager. Our new associates are placed with experienced management staff to be taught
the application of their duties on the job. UNISERVE will also take this time to evaluate all current
and existing cleaning personnel for re-training with their job duties.

Cross Training will take place at the discretion of the supervisor and/or manager.
During the initial sixty (60) days of contract implementation, the supervisors will evaluate all
workloads and make changes accordingly. Cross training will allow the multi-dimensional training
of employees allowing for more flexibility to cover shortages and/or improve training efficiencies.
In-Service Training our in-service training is the on-going component that refreshes basic skills,
adds technical and advanced skills,and ensures that our associates are capable of meeting quality
and productivity services.
Level I
This is the foundation of a basic cleaning program. In-
depth information on the different types of cleaning

Cleaning and Machine Equipment Care & Storage demonstrates proper procedures for loading
custodial carts, proper care of expensive power equipment and managing the custodial closet.
Shows how these preparations improve efficiency, simplify custodial tasks and help avoid
subsequent errors.
Basic Janitorial Cleaning covers common cleaning methods in interior and exterior building
facilities. Different types of surfaces are covered on identifying the correct cleaning procedures
including how to police and inspect areas and identify soils and stains. Trash removal, sanitation,
stains & cobweb removal, sand urns cleaning and manual cleaning techniques such as spot
cleaning & sweeping.

Dust & Damp Mopping explains the steps to remove dust and light litter or soil from floors as a
daily cleaning procedure. It also must be used as the first step in preparation for more extensive
floor maintenance work.
Restroom Cleaning teaches the proper method of cleaning restrooms, precautionary signs and
barricades, odor control, drain maintenance and loading of dispensers, trash removal, basin
sanitation, sanitizing and disinfecting commodes and urinals, spot clean walls, ledges, vents and
partitions.
Window cleaning, venetian &vertical blinds, and partitions describe ways to clean mirrors, glass
and the surrounding areas as well as bright work.
Level ll
Provides a more advance cleaning program that
demonstrates daily maintenance procedures for
` carpeted, resilient floors and walls respective to

Floor ID identifies the different types of hard
flooring (such as terrazzo, marble, stone, vinyl,
resilient and concrete) and carpeting including manufacturers recommendations and
precautionary signs and barricades.
Floor Scrubbing & Stripping demonstrates all the equipment, supplies and procedures to
professionally strip and recoat floors. Review bulk liquid and chemicals,complete floor machine
operation and maintenance (Including pads&brushes),strip and rinse,and new finish application.
Carpet Care features a through preparation of professional maintenance through Vacuuming,
Bonnet, Dry Chemical and Dry Foam and Extraction methods.
Floor Buffing describes the before and after steps with dust and damp moping and goes into the
different techniques in floor buffing with power machines and buffing pads.
Resilient Floor Care teaches excellent procedures for maintaining high-gloss,scuff-free,hard floors
with crucial steps(regular cleaning, removal of old wax, waxing, linoleum care and cleaning,vinyl
no wax floors)to achieving this are highlighted in the program.
Pressure Washing provides professional exterior hot water pressure cleaning. Learn techniques to
remove hard stains on exterior buildings,sidewalks,storefronts,brink/stone,painted surfaces,and
graffiti.
Upon completion of Level I &II sessions,employees are qualified to become professional cleaners
who have the knowledge to operate power equipment, use of chemicals and safety procedures
and guidelines.

Spec Level Ill is the only session requiring prerequisite of
Level I & II.
Employees will understand the Supervisors role as in planning,organizing, scheduling, supervising,
assigning, and evaluating the work of a janitorial staff. Level III reviews how to establish daily and
non-routine schedules, work assignments and standard procedures for work order requests.
Understand purchasing platforms for equipment and supplies. Understanding the Quality
Assurance programs, quality management reviews, and budgets. Learn how to inspect job sites to
determine work required and evaluate job performance and prepare a variety of records and reports
including time cards, worksheets, accident reports, and maintenance.

UNISERVE utilizes janitorial equipment that maximizes effective reduction of
building contaminants with minimum detrimental environmental impact and
o energy use, including vacuum cleaners, hot water extraction equipment,floor
buffers, burnishers, and automatic scrubbers. We strive to have cleaning
ENERGY STAR activities be in compliance with Green Seal's GS-42 standard for Cleaning
Services, including use of GS-37 cleaning supplies.
Furthermore, UNISERVE will be utilizing WAXIE Sanitary Supply's Flexible Dispensing Unit which
would reduce the number of plastic bottles and control the proper dilution of chemicals resulting
in a sustainability best practice.

information on existing cleaning programs,including their cost and quality ratings,to analyze and
evaluate our services. Along with these best practices, UNISERVE considers our clients special
needs and their existing infrastructure to redesign a cleaning program that provides customized

appropriate staffing levels, tasks, procedures, and frequencies. Using this information,
management and supervisors have improved systems to achieve better performance, increased

UNISERVE's operational approach to time management encompasses a sophisticated time
keeping system. All of our schedules are preloaded Into the software and throughout the day,
the timekeeping machines communicate with the software. Should anyone fail to clock in or
clock out within ten minutes of shift, the system will notify the manager and suggest possible
backup personnel and their availability. Furthermore, UNISERVE maintains a pool of cross
trained employees on all shifts at all times that will provide coverage until a replacement
arrives.
We will be utilizing the NOVAtime 4000 software in conjunction with Ingersoll Rand Biometric
HandPunch 1000-E.
NOVAtime 4000 STAR is the self-hosted, licensed version of the SAS state-of-the-art, web-
based workforce management system that has raised the bar both technically and functionally.
Accessible through the Internet, this system makes it easy and convenient to monitor our
workforce and control labor costs. High-level functions are provided to help our management
team analyze real-time work data to affect dramatic increases in efficiency and productivity.

CUSTOMER SERVICE
UNISERVE will provide adequate supervisory staff as required to maintain the highest level of
cleanliness. UNISERVE through its quality assurance plan will establish, in conjunction with the City
of Rosemead performance metrics to be utilized in measuring our cleaning program and its
effectiveness.
UNISERVE determines customer satisfaction through its successful deployment of services as well
as through its provision of value-added services. UNISERVE develops and/or reviews service
specifications for the most efficient deployment. UNISERVE audits its work performed for further
enhancement. UNISERVE tracks data of our customer's needs and satisfaction as an integral tool in
providing first-rate customer services. UNISERVE tracks performance level data, customer service
evaluations, monthly inspections and inspection audits to ensure continuous service excellence
throughout every shift meeting and exceeding client's service level expectations.
UNISERVE has established a Customer Service Center (CSC) to
centralize all customer support needs.Although the main purpose
I 4. x. of this call center is to receive and document customer work
requests,the CSC provides access to customers to all of the Service
f'. Center resources at hand which include conference calling
capabilities as well as an Emergency Hotline.
Every call that is received by our CSC will be keyed into our customer service database. Once that
occurs, the request will be tracked by the system from routing it to the proper specialist to
confirming the resolution within the expected time frame.
The CSC is equipped with state of the art communication technology. Through this system, our
CSC staff is able to monitor the progress of all service requests and works in progress.
It is our mission to use the technology available today to provide every one of our client's real time
data on service requests, resolutions, etc.
UNISERVE holds the unique distinction of being ISO 9001 registered.
Under these strict guidelines, UNISERVE is required to implement and
standardize optimal processes—providing detailed "how to"
Egl information for our employees.This process is managed by a dedicated
supervisor at each location.

UNISERVE will be utilizing Cleantelligent Software to manage the City of Rosemead's quality
assurance program. In addition to our other quality processes, UNISERVE will utilize this software
to track internal and external work evaluations. Upon contract award, UNISERVE will upload to
Cleantelligent the quality metrics the City of Rosemead and UNISERVE agree upon. From that
point, either UNISERVE or the City of Rosemead representatives can log on to the cloud based
system via the internet, an iphone application or an android application.
Once the system is accessed,the City of Rosemead's inspection form will be available to use. This
form will include all areas and the scope of services to be inspected. Furthermore, photos will be
allowed to be uploaded and stored for reference.
Once an inspection is completed, the quality score will be tabulated and any underlying work
orders will be routed to the Customer Service Center (CSC).
All scores are recorded and the City of Rosemead will be provided performance metrics history and

At UNISERVE we have centered our company goals on SUSTAINABLE PRACTICES and GREEN
CLEANING because we are passionate about being part of the solution. Our passion is not only for
the environment, but for the health and wellness of the adults and children who come in contact
with the chemicals and processes used during cleaning every day. UNISERVE is committed to
Sustainability and Cleaning for Health as a strategic objective.
All of our cleaning processes and products are consistent with the US Green Building Council's
(USGBC) and Leadership in Energy and Environmental Design (LEED) housekeeping requirements
for green building certification. Our extensive experience in Green Cleaning has enabled us to
become consultants to our clients in assisting them with all their green and sustainability
initiatives.

All of our disposable paper products come from 100% recycled fiber and we utilize recycled mop
heads which are manufactured from recycled plastic bottles.
In addition to our disposable products, UNISERVE whenever possible will utilize tools and
equipment manufactured with recycled products, such as,our mop handles which are made from
bamboo, a more quickly replenished resource than standard wood.

UNISERVE believes it is necessary to keep the client abreast of project development and concerns
on a regular basis and provides its clients with customized reports to fit individual client needs and
specifications. These reports provide crucial data, which help identify trends in efficiency.
Managers can easily run reports on key performance indexes, service reports and schedules,
thereby providing crucial efficiency summaries without tedious manual calculations. These
techniques and systems improve processes by using data-based management rather than instinct
and hunches. This sort of tight data tracking coupled with the intelligent control of process
systems aid our clients in significant cost savings.
UNISERVE will provide the City of Rosemead access to our Cleantelligent maintenance
management software cloud. Through this site,the city will be able to view real-time work order
and quality control processes. In addition to online access, UNISERVE will provide customers with
a monthly analysis of all agreed upon performance metrics.
Within ten (10) days of contract award, UNISERVE will meet with the City of Rosemead
representatives to compile performance metrics which will be utilized on this project. Once
performance metrics have been established,daily and weekly inspections will be compiled as the
contract progresses. The City of Rosemead will be provided weekly reports showing any service
deficiencies uncovered, our action plans and status of completed and open work orders.
Contract agreement billing is input via our Finance dept. that is responsible for the monthly
invoicing process and runs edit report to insure accuracy. Any billing which is outside the
contractual billing is supported by work orders and reviewed by the local manager and contract
administrator prior to being sent to Finance for completion.

BID UNIT PRICES
I. The contractor agrees that for requested and/or required changes in the scope of
work, including additions and deletions on work not performed, the contract sum
shall be adjusted in accordance with the following unit prices.
II. Contractor is advised that the submitted unit prices will be used as one of the
determining factors in the contract award. Unreasonable prices may result in
rejection of the entire bid proposal. Unit prices listed below refer to all services
provided, including but not limited to, materials, labor, overhead, and profit for the
contractor.
Ill. The unit price quoted by the contractor shall be those unit prices that will be charged
or credited for labor and materials to be provided regardless of the total number
units and/or amount of labor required for added or deleted items of work.
IV. All work shall be performed in accordance with specifications or otherwise herein
specified. Workmanship shall be in accord with the best standard practices.

Note: The City of Rosemead reserves the right to reduce or increase the quantities of any items
in the schedule of bid items above, within the limits define in Section 3-2.2.1 of the Standard
Specifications. to stay within the budgeted amount of this project.
The undersigned agrees that these Contract Bid Forms constitute a firm offer to the Owner
which cannot be withdrawn for the number of Working Days indicated in the Request for
Proposals from and after the bid opening date, or until a Contract for the Work is fully executed
by the Owner and a third party, whichever is earlier. The undersigned also agrees that if there
is a discrepancy between the written amount of the Bid Price and the numerical amount of the
Bid Price, the written amount shall govern.
If awarded a Contract, the undersigned agrees to execute the formal Contract, which will be
prepared by the Owner for execution, within five (5) Calendar Days following the Letter of Award
for the Contract. and will deliver to the Owner within that same period the necessary original
Certificates of Insurance, Endorsements of Insurance, Performance Bond, and all other
documentation and certification required by the Contract.
The undersigned offers and agrees that if this bid is accepted, it will assign to the purchasing
body all rights, title, and interest in and to all causes of action it may have under Section 4 of the
Clayton Act (15 U.S.C. Section 15) or under the Cartwright Act (Chapter 2 of Part 2 of Division 7
of the Business and Professional Code), arising from purchases of goods, materials or services
by the Bidder for sale to the purchasing body pursuant to the bid. Such assignment shall be
made and become effective at the time the purchasing body tenders final payment to the
Contractor.
Bidder understands and agrees that, when requested by Owner, he shall provide: (1) evidence
that the person signing this Bid is authorized to bind Bidder to this Bid and to a contract resulting
therefrom; and (2) any other information and documentation, financial or otherwise, needed by
Owner to award a Contract to the lowest responsible and responsive bidder.
