TRIPEPI, CITY MANA....9pLigsetil DATE: AUGUST 19, 1997 RE: REQUEST FROM THE GAS COMPANY FOR A BLANKET EXCAVATION PERMIT The Gas Company has contacted staff requesting a blanket agreement for excavation permits. The blanket permit will allow the Gas Company to perform work that will cover: • Isolated excavations 3' x 5' or less. • New business main packages where the excavations are 3' x 5 or less. • Installation and replacement of service lines two inches (2") or less in diameter. Main line installation excluded. • Required potholing for Gas Company activities and City projects. • Barholes to confirm leakage or to perform depth checks. The Gas Company will submit the locations for proposed work to the City by facsimile for assignment of the permit sub-number. The City will return the list by facsimile with the assigned sub-number prior to work beginning. The process will allow the permits for each location to be issued without the need of individual permit forms completed and signed by the Gas Company. The sub-number assigned will allow the City to monitor the locations of the work in the same manner that is currently in use. Additionally, the City can suspend or revoke the blanket permit at any time if Gas Company supplies false information or violates any condition or provision of the blanket agreement. RECOMMENDATION It is recommended that the City Council approve the blanket permit and authorize the Mayor to sign on behalf of the City. FGT:fw Rptcty\gascoper.mit CITY OF ROSEMEAD BLANKET EXCAVATION PERMIT GENERAL Excavations permitted under the blanket permit process shall be assigned a sub-number by the City of Rosemead for each excavation. Each sub-number may be used for a maximum of one excavation, 3' x 5', or less, and including the resultant permanent paving repair. The Gas Company will transmit by facsimile a list of proposed excavations to the City of Rosemead for sub-number assignment as needed. The City of Rosemead will return by facsimile the list with assigned sub-numbers within one working day of receipt. No work will commence prior to sub-number assignment, except for emergency situations. A Monthly Maintenance Blanket Permit Activity Report (Report) (Exhibit A) shall be provided to the City of Rosemead by the end of the first working week of the following month. Such Report will cover activities from the first day to the last day of every month. Permits will be posted after permanent paving repairs have been completed. Information to be provided for each excavation will be the assigned sub-number, location and purpose of excavation, size and type of surface cut, and date completed. The Gas Company's monthly fee to the City of Rosemead will be based on the total costs indicated on the Monthly Maintenance Blanket Permit Activity Report. In addition to the Report, a Daily Line-Up (Exhibit B) will be faxed to the City of Rosemead on a daily basis. Blanket permit application shall be renewed annually on a calendar year basis. The City Engineer may, in writing, suspend or revoke this Blanket Excavation Permit, at any time, whenever the Permittee has supplied false information or has violated any law, regulation, or a condition or provision in this document. Barholes to confirm leakage or to perform depth checks. Page 1 of 5 TRAFFIC REQUIREMENTS 1. If no special traffic provisions are required, one 12-foot lane of traffic must be maintained in each direction. Permittee shall furnish and place all advance warning signs, any other flag control devices and flag persons needed to make the job site - safe for the traveling public or any other devices required by and as directed by the City of Rosemead Public Works Inspector. All advance warning signs shall be constructed and placed in conformance with the current requirements as specified in the Work Area Traffic Control Handbook (WATCH) manual, prepared by the Southern California Chapter of the American Public Works Association. 2. Clearance from traffic lanes shall be a minimum of five feet (5') to the edge of any excavation and two feet (2') to the face of any curb, pole, barricade, delineator, or other vertical obstruction. 3. Adequate advance warning barricades must be maintained and traffic routed safely around open excavations and obstructions. Excavations adjacent to or across traveled lanes must be backfilled, compacted, and temporarily or permanently paved before the end of each working day. 4. The permittee shall place and maintain barricades and warning lights at each end of the excavation until the excavation is entirely backfilled and surfaced. 5. Traffic shall not be detoured across a double yellow line, raised or painted medians, or left or two-way left turn lanes without prior approval of the City of Rosemead Public Works Inspector. 6. Any traffic marking or curb marking obliterated or removed by the operations of the permittee will be replaced by the permittee by the end of each working day. Temporary markings will be allowed with prior approval of the City of Rosemead Public Works Inspector. 7. Permittee may post temporary "NO PARKING SIGNS". Signs shall be posted at least 24 hours, but no more than 48 hours in advance of the work. Permittee shall provide the signs and be responsible for adding the dates and hours of closure to the signs. 8. For any construction in the traveled way which results in traffic flow restriction, the permittee shall at all times provide access for emergency vehicles and residents, and shall notify the L. A. County Fire Department and Sheriffs Department prior to commencement of work. Page 2 of 5 PROTECTION OF UNDERGROUND FACILITIES 1. An excavation shall not be performed until permittee receives a "ticket number' from "Underground Service Alert", telephone number 1-800-422-4133 and said "ticket number" is entered on the face of the order by the permittee. "Underground Service Alert" requires a minimum of 48 hours notice prior to the beginning of excavation to verify the location of pipelines containing hazardous substances. 2. In addition, the permittee is responsible for thoroughly researching the records to determine if other non-hazardous substance lines are in the area in which he intends to excavate and to notify the owners of such lines 48 hours prior to excavating to allow them to identify and determine the alignment of their lines. Immediately prior to excavating, the permittee shall arrange with the company to locate or expose its facilities. INSTALLATION OF NEW OR RELOCATED UNDERGROUND FACILITIES The following minimum depths of cover below existing, finished, or proposed gutter grade shall be required for all underground pipeline within the roadway area: 1. 24" for service pipelines. 2. 30" for all other pipelines. MISCELLANEOUS Unless backfilled, excavations shall be adequately and safely protected at all times with one or more of the following: steel plates, approved fencing with barricades or watch persons. All work shall be in conformance with all applicable safety laws. For information regarding construction safety orders, contact the local office of the State of California, Department of Industrial Relations, Division of Industrial Safety. All bridging must be approved by the City of Rosemead Public Works Inspector. If steel plates are used, the minimum surface bearing width shall be 12"on side of the excavation, and the following minimum provisions shall apply for legal loads. 1. 3/4" x 50" x 410" wide steel plates may be used to a 70" maximum span length. Page 3 of 5 2. 1" x 816" x 4'0" wide steel plates may be used to a 4'6" maximum span length. A minimum of 90% relative compaction is required on all backfill. Costs for inspection shall be borne by the permittee. Permittee shall notify the City at least 24 hours in advance of any excavation and for any permanent paving repairs, except for emergency situations. Modification of the above requirements and/or additional requirements may be made in the field by the City of Rosemead Public Works Inspector. For the purpose of applicable permit fees under the City of Rosemead Municipal Code, each individual sub-number will be considered a separate permit number and billed as appropriate. PAVING Paving will be in accordance with the City of Rosemead requirements existing at the time of excavation repair. INDEMNIFICATION Permittee shall indemnify, defend and hold harmless the City of Rosemead, its officers, agents and employees, from and against any and all liability, suits, actions, proceedings, judgments, claims, liens, losses, damages (whether in contract or in tort, including personal injury, accidental death or property damage), costs and expenses (including attorney's fees, litigation and arbitration expenses) which arise from or are caused by, or which are alleged to have arisen from or to have been caused by, any and all acts or omissions, under the permit for excavations, of permittee, its officers, agents, employees, contractors, subcontractors, or by anyone it directly or indirectly employed, whether the losses, injuries or damages shall occur or be discovered before or after permittee's use of, or permittee's excavation, construction, installation and/or operations in or upon, the City's properties, premises or facilities. Page 4 of 5 ACKNOWLEDGMENT The permittee has read and reviewed this document (City of Rosemead Blanket Excavation Permit), all pertinent City of Rosemead ordinances, regulations and policies, and Underground Service Alert requirements. The Permittee agrees to comply with and be bound by all the terms, conditions and provisions in this document.
