CORE Overview
We are a crisis response organization on a mission to empower underserved
communities in and beyond crisis.
We are working alongside Herald Christian Health Center (HCHC) to provide
inclusive, contextually appropriate public health outreach to communities
within the San Gabriel Valley (El Monte, South El Monte, Rosemead).
Our goal
is to
focus on preventative
measures
that
holistically
address the
causes
of
ailment
which
are specific
to
members
of
a
community.
Examples include vaccinations,
health screenings, lifestyle
assessments, mentalhealth, etc.

Herald Christian Health Center
Herald Christian Health Center (HCHC) is a Federally Qualified Health
Center that provides medical, dental, vision, acupuncture, and behavioral
health services.
We have been serving the San Gabriel Valley since 20 07, and have locations
in El Monte, Rosemead, San Gabriel, City of Industry, and San Diego. Our El
Monte location
also provides
assistance with
Medi -Cal, CalFresh, and WIC
enrollment,
as well as
linkage
to additional resources.

Program
Our program shares leadership with the community to identify
local health priorities, to reduce gaps in health outcomes, anc
improve
being.

A Visit each household
Gather and analyze data on health, mental health, and social
determinants of health
A� Assist with referrals and linkage to healthcare and social services
� Convene the community and develop community action plans
Mobilize partners to advance health equity and improve conditions

The Household Assessment is a survey tool
developed by the Los Angeles County
Department of Public Health to help us
understand what your needs are.
o It contains questions on
housing/neighborhood info,
community concerns, social/economic
needs, demographic/ household
characteristics, and health/mental
health conditions.

Social Determi"nants
of Health
Many things affect your health and well-being. These include having
home, transportation, understanding the language, and access to
food, education, and job opportunities.
We call these things "Social Determinants
of Health." Our goal is to understand what
resources you and your community need
and help connect you to them.
