According to our data collected for week 5/13/2024 to 5/17/2024, we
made 37 engagements of which S were first time engagements, 29
follow up engagements with 5 engagements refusing supportive
services.

• Team linked 1 individual to interim housing services located at Soul
Housing.
• Team linked 19 individuals to food pantry services located at
Shepherd's Pantry and East San Gabriel Valley Coalition.
• Team provided 17 housing navigation services.
FUTURE GOALS
• Follow up with client regarding consistent engagement with mental
health provider.
• Focus on housing clients through DMH.
• Encourage client to accept SUD Treatment.
