STATS OVER THE PAST WEEK
According to our data collected for week 6/3/2024 to 6/7/2024, we made
31 engagements of which 6 were first time engagements, 25 follow
up engagements with 4 engagements refusing supportive
services.
HIGHLIGHT
• Team linked 4 individuals to interim housing at: Soul Housing 0),
Dave's Housing 0), Safe Landing (1) and Volunteers of America
Lancaster 0).
• Team linked 1 individual to SUD treatment at American Recovery
Center.
Team assisted 2 indivduals with obtaining CA identification.
FUTURE GOALS
• Follow up with client regarding consistent engagement with mental
health provider.
• Focus on housing clients through DMH.
• Encourage client to accept SUD Treatment.
