Construction Co.I 1 1 r , nr BIDDER: '57' L1~~~11 W SECTION 1 -BID SCHEDULE CBF-0 CITY OF ROSEMEAD ANNUAL CURB RAMP PROJECT FEDERAL REQUIREMENTS The City of Rosemead ANNUAL CURB RAMP PROJECT is subject to the Federal prevailing (Davis-Bacon) wage requirements and related labor standards. The primary contractor is required to comply with all requirements imposed by the Davis-Bacon labor standards, including but not limited to the following: 1. The primary contractor acknowledges and accepts that this project is a Federally assisted project and Davis Bacon will be enforced. 2. The primary contractor acknowledges and accepts that the Federal Labor Standards Provisions HUD Form 4010 is a part of this contract. 3. The primary contractor is aware that he and his company must be eligible to participate on a Federally assisted project and cannot be on the "Lists of Parties Excluded from Federal Procurement or Non-procurement Programs", otherwise known as the Debar List. NOTE: It is the primary contractor's responsibility for checking all sub- contractor's eligibility and ensuring that they are not on the debar list. 4. The primary contractor acknowledges and accepts that it is his responsibility to ensure that the Federal Labor Standards Provisions (HUD Form 4010), wage rates and the same language as listed in the contract between the City of Rosemead and the primary contractor, is included in all subcontracts. Inclusion: by reference is unacceptable. 5. The primary contractor acknowledges and accepts that all records, including payroll, must be kept for three (3) years after completion of project. After the three years have passed and before any records can be destroyed, the primary contractor must contact the City of Rosemead, Community Development Department, to ensure that the records can be destroyed.
